Providence Oregon CEO Discusses Potential Reversal of Access Restriction to Specialty Care
A recent development has sparked concern among low-income residents of Portland, Oregon, as the city's healthcare landscape undergoes a significant shift. This change occurred due to a Medicaid insurance network adjustment that took effect this month.
CareOregon, the organization managing the affected plan, asserts that its extensive provider network is sufficient to accommodate the necessary adjustments. However, some individuals express doubts and warn of potential longer wait times.
In an interview with WW, Jennifer Burrows, the CEO of Providence Oregon, offered an intriguing perspective. She suggested that the health system and the insurer might reconsider their decision or modify the arrangement if CareOregon's insurance network encounters challenges. Burrows stated, 'We've agreed with CareOregon to have a new conversation if they indeed feel they are struggling with network adequacy.'
CareOregon operates the largest Medicaid plan in the Portland area. Members of this plan often receive primary care at federally qualified health centers. In the past, if members required specialist care, such as neurology, oncology, or psychiatry, Providence's extensive network was an available option, alongside specialists from other institutions like Legacy Health and Oregon Health & Science University.
On February 15th, new referrals to most of these Providence specialists were no longer an option for the plan's over 200,000 members. This development highlights the potential impact of financial strain and rising healthcare costs on Portlanders' access to essential medical services.
Burrows explained that certain exceptions remained for members of the affected CareOregon plan. Members can still receive immediate care at Providence hospitals and urgent care. Specific referral pathways, such as those to Providence OB-GYNs and pediatricians, remain open. Additionally, CareOregon Medicaid patients already under the care of a Providence specialist can continue their treatment.
However, in general, if the over 200,000 CareOregon Oregon Health Plan members in Portland require specialist referrals, they will need to seek care elsewhere. Burrows acknowledged the potential impact on individuals, stating, 'We know that it has an impact on some people.' She also assured that they have a plan to ensure that no current patient's care will be interrupted.
Furthermore, Burrows noted that Providence specialists remain accessible for referrals from the approximately 60,000 Portland area Medicaid members enrolled in Providence's in-house insurance plan. Kaiser Permanente and OHSU also offer smaller-scale Portland area Medicaid plans, while Legacy Health's plan was integrated into CareOregon's network last year.
Burrows emphasized that the network change was a 'joint decision' made with CareOregon, as both entities strive to deliver high-quality services to Oregonians more efficiently. She explained, 'As we worked with CareOregon, they felt they had a large enough network of specialty care without our Providence specialists, and they believed they could meet their members' needs. We engaged in numerous discussions, shared extensive data, and identified areas of concern regarding network adequacy. Those areas have been kept open.'
